The checklist below should help you gather the necessary materials together to apply for an account. If you are new to the art glass business or have any problem supplying an item listed below, please call our sales department at 800-525-0940 for assistance.

Please provide a SIGNED copy of
EXEMPTION CERTIFICATE (above),
and one or more of the following:

Current resale tax number (Business license for states not requiring & Canada)

Federal ID number (or Social Security number)

Voided business check

Two business cards

Samples of business marketing tools

Names and address on all documentation must match and
the business must be in the art glass or related field.
