Your application must include:

 A SIGNED copy of
EXEMPTION CERTIFICATE (above)

 Current resale tax number
(Business license for states not requiring & Canada)
We will need a hard copy of the license mailed,
faxed or emailed

 Samples of Business Marketing Tools
(for example, website, business cards, yellow page ad)

 Voided business check




Names and address on all documentation must match and
the business must be in the art glass or related field.
